Dear Sir or Madam:
I am a victim of identity theft. An identity thief used my personal information to open an account with your company. I did not open or authorize this account. Please close this account immediately, absolve me of all charges on the account, and take the appropriate steps to remove information about the fraudulent account from my credit files.
I have enclosed a copy of my Identity Theft Report and a copy of my credit report that shows the fraudulent account with your company. I have also enclosed a copy of the Notice to Furnishers of Information issued by the Federal Trade Commission. The notice explains your responsibilities under the Fair Credit Reporting Act. It explains that when you get a consumer’s written request and Identity Theft Report, you must stop giving information that results from identity theft to any credit reporting company. The Notice also explains that when a credit reporting company notifies you that information you sent it may be a result of identity theft, you must stop sending that information to any credit reporting company, and be sure you don’t try to sell or transfer the fraudulent debts to another party for collection.
Please close the account, absolve me of all charges, do what the Fair Credit Reporting Act requires, and send me a letter explaining your action.
